If you require a routine home visit please contact the surgery before 10.45am.
Any routine home visit requests after 10.45am will be dealt with the next working day.
Sharing Medical Records
The NHS has launched Shared Care Records. This enables us, your GP practice, to share your information with other organisations i.e Hospitals, Physiotherapists, District Nurses etc. This will help improve the care you receive.
All patients are automatically “opted in” to having their records shared. If you do not want your medical records sharing with other medical organisations please complete the form at the surgery.

A carer is someone who looks after a friend or relative who needs help because of physical illness/disability, mental health or learning difficulties
Are you a carer? YES NO
Are you a “young carer”? YES NO
If “yes” would you like us to record this in your notes? Recording that you are a young carer will not affect the treatment you or the person you care for at the surgery receive and will not be disclosed to outside organisations.
If you need to be given confidential information about the person you care for you will need to ask for the confidential information form that will allow us to share this information with you.
Please ask at reception for a carers registration form and return it to reception when you have completed it
